Insurance

Rates will be available at registration. The school
district will provide special insurance coverage for all
students participating in extra-curricular activities with the
exception of football. Please be advised that the policy
provided for extra-curricular activities is supplemental
coverage and should not be considered the student’s main type of
insurance. Students will be required to provide proof of
insurance prior to participating in an activity. |
HEALTH EXAM AND IMMUNIZATION INFORMATION
A
health examination and proof of immunity against diphtheria,
pertussis, tetanus, polio, red measles, mumps and rubella for
all Freshmen and
out-of-state students must be presented before a student may
register for classes. Medical forms can be obtained from the
high school office or unit office or
downloaded and printed here. |
FEE WAIVERS AND FREE/REDUCED LUNCH
Fee waiver
and free/reduced lunch forms will be sent home with registration
letters. It is very important that you bring completed forms
and proof of income or your “Direct Certification Letter”
from the Illinois State Board of Education with you on the
appropriate day you register. |
