Transcripts for persons attending school at Carterville Unit School District #5 are kept for 60 years from the last date of attendance.

After the 60 years has passed, you may request your original transcript.  Please email Becky Anderson with this request.

If you need to request a copy of your transcript prior to the 60 year requirement, please email Kim Spencer at CHS.  The form required for requesting a copy of your transcript can be found on the CHS web site.
